[oc] VGA/LCD Controller

Hi All,

A new version of the VGA/LCD controller is available from CVS.
This new version contains some major bug fixes in the Wishbone Master and 
ColorProcessor modules.

Also included is a new top-level and sub-level (vga_and_clut.vhd & 
csm.vhd). These two files add the color-lookup-table to the VGA core (i.e. 
on-chip CLUT). Ram generation has been tested with Altera and Xilinx parts.
